Clinical Consent Form
Edel Dietitian – Client Consent Agreement
Effective Date: 1st May 2026
This agreement applies to all clients receiving dietetic services from Edel Dietitian, owned and operated by Edel Warke (“Practitioner”).
1. Nature of Services
Dietetic services provided include:
- Evidence-based nutrition assessment
- Personalised dietary advice
- Behavioural and lifestyle guidance
- Ongoing monitoring and support
These services are based on current scientific evidence and professional judgement.
2. No Medical Diagnosis
You acknowledge that:
- Dietetic services are not a substitute for medical care
- The Practitioner does not diagnose medical conditions unless explicitly qualified and permitted by law
- You are responsible for consulting your GP or specialist where appropriate
3. Client Responsibility
You agree that:
- You will provide accurate and complete health information
- You will inform the Practitioner of any changes to your health or medication
- You are responsible for implementing dietary advice
Outcomes depend on individual adherence and physiological factors.
4. Informed Consent
By engaging in services, you confirm that:
- You understand the nature and purpose of the consultation
- You have had the opportunity to ask questions
- You consent to receiving dietetic advice voluntarily
5. Risks & Limitations
You understand that:
- Nutrition interventions may involve dietary changes that require adjustment
- There is no guarantee of specific outcomes
- Results vary between individuals
6. Confidentiality
All personal and health information:
- Will be treated confidentially
- Will only be shared where required by law or with your consent
- Will be handled in accordance with applicable data protection laws
7. Data Use
Your data may be used to:
- Deliver dietetic services
- Maintain clinical records
- Improve service quality

8. Payment, Cancellation and Rescheduling
- Fees must be paid in advance unless otherwise agreed.
- Clients may cancel or reschedule without charge by providing at least 24 hours’ notice.
- Cancellations or rescheduling requests made with less than 24 hours’ notice are non-refundable. Missed appointments are treated as no-shows and are non-refundable.
- Exceptional circumstances, including illness, may be considered at Edel Dietitian’s discretion.
- If Edel Dietitian cancels an appointment, the client may choose a full refund or reschedule without additional charge.

9. Limitation of Liability
To the fullest extent permitted by law:
Edel Dietitian and Edel Warke are not liable for:
- Outcomes resulting from client decisions or non-adherence
- Adverse effects where advice has not been followed correctly
- Indirect or consequential losses
10. Consent Declaration
By proceeding with services, you confirm that:
- You have read and understood this agreement
- You voluntarily consent to dietetic services
- You accept full responsibility for your health decisions
